The Champlain Community Support Network (CCSN) is a bilingual collective of non-profit Community Support Service (CSS) providers across Eastern Ontario.
What Does
CCSN Do?
Through strategic networking, partnerships, and collaboration, we mobilize the collective strengths and experiences of the CSS sector to build capacity, to innovate, and to drive positive change across the healthcare system.
Together we support member organizations whose work ensures that older adults and adults living with disabilities are empowered and feel supported to live independently and safely at home. CCSN strives to achieve the vision and mission that no one organization can accomplish on its own.

We support Community Support Service providers through strategic networking, partnerships, and collaboration, we mobilize the collective strengths and experiences of the CSS sector to build capacity, to innovate, and to drive positive change across the healthcare system.

Many of our services are free of charge while some services require a small fee. We work with each client to ensure they can access our affordable programs and services.
The E-Referral Network helps you to connect with services for seniors and adults with disabilities available through trusted not for profit providers near you. The E-Referral Consortium supports improved system navigation and secure electronic referral to and from Community Support Services organizations. Any member of the public, healthcare, or community support service provider can make referrals to community support services electronically at https://www.communityhomesupport.ca/find-services
Caredove:
Visit www.caredove.com/champlaincss to connect patients to Community Support Services. No account is required to send a referral however if you would like to receive a follow up about a referral you will need to set up a free account by selecting “Become a Referrer” located at the top right hand corner of the screen.
OCEAN:
Find us on OCEAN under “Central Intake Community Support Services (Region)” Referrals will be assessed and forwarded to the most appropriate community support service organization(s).
Membership is available for all not-for-profit CSS provider organizations who are funded by Ontario Health under a Multi Service Accountability Agreement (MSAA) within Ontario Health East. CSSN strives to achieve the vision and mission that no one organization can accomplish on its own.
